What a supplier evaluation is
A supplier evaluation (แบบประเมินผู้ขาย) is the periodic review of a vendor your organization already buys from: did the goods meet specification, did deliveries arrive on time and complete, were prices and terms honored, how did they respond when something went wrong, and did the documents (COA, SDS, warranties) arrive with the goods. One form covers one supplier for one period, and it ends in a status, never in a bare number.
Evaluations are what quality systems expect as a matter of course: ISO 9001-style systems ask that external providers be evaluated, re-evaluated on a cycle, and that records of the results exist. An audit that asks "how do you select and monitor suppliers" is answered by this form and the register it updates, with dates.
Supplier evaluation vs price comparison
The two documents are cousins and are often confused. They answer different questions:

A price comparison looks forward at offers; an evaluation looks back at delivery. A vendor who wins comparisons but fails evaluations is quoting well and performing badly, which is exactly the pattern the two documents together expose.
The five criteria, and where each score comes from
Every criterion on the form names the record to score it from, because unanchored scores collect feelings:
Quality
Did goods meet specification, with few rejects? Score it from the goods-receipt forms: the accepted and rejected columns are the count, not a memory.
Delivery
On time and complete? The gaps between ordered, delivered and accepted quantities on the receipts are the record.
Price and terms
Did invoices match quotations, and were credit terms honored? Score from the quotation and invoice pair.
Service and responsiveness
How fast were questions answered and problems fixed? The issue log holds the incidents and how each closed.
Documents
Did COA, SDS and warranty papers arrive with the goods, unchased? The receipt checklist and the document file answer this.
Three habits that keep evaluations honest

Score from records
Open the goods receipts and the issue log before scoring, not after. An evaluation written from memory rewards whoever called most recently.

Evaluate on a cycle, and on trigger
A fixed cycle (yearly in many companies) plus a re-evaluation whenever problems repeat between cycles. The next-due date on the form is what makes the cycle real.

End in a status, then update the register
Tick one of the three decisions and carry it to the vendor register the same day. A conditional result always names what is being watched and when it is re-checked.

How often should suppliers be evaluated?
On the cycle your organization sets, commonly once a year for active suppliers, plus an out-of-cycle re-evaluation when problems repeat or after a serious incident. The form carries a next-evaluation date so conditional results get re-checked instead of forgotten.
